Official Application Form:

The next step to take when you are applying for a Florida card is to visit the local courthouse for the application form. If you have already applied for a new Florida health insurance plan, you should go back to the same pharmacy that issued your new application and purchase the insurance right away. Make sure that you provide the same information about your existing plan as you do for your new application. This way, it will be easier for the pharmacist to help you obtain your new insurance.

Complete Your Enrolment Process:

Once you are finished applying for your new Florida medical card, you should then complete your enrollment process. The next step involved in this process is to go back to the pharmacist who issued your previous card and sign the information given to you on the application for the medical card. You will need your identification card number, social security number, and the prescription’s name (if you have one) in order to complete your enrollment.
